Elder Care Concierge offers a four-phase model to address the unique needs of aging individuals: Urgent, Planning, Routine, and Legacy. We invite you to explore each phase to see how we can support you and your loved ones through the aging process with confidence and care.

The Urgent Phase focuses on quickly addressing immediate aging-related challenges, ensuring safety and well-being.  We begin with our nursing staff’s diligent assessment considering housing, care, transportation, communication, and medical advocacy needs.  This phase results in a a detailed report outlining best options and solutions.  Typically this phase takes 1-3 months, but we aim to go at your pace.

The Planning Phase aims to prepare for future needs by discussing legal, insurance, and end-of-life planning, creating a methodical 5-year plan with contingency strategies.  This phase starts after the Urgent Phase has been completed.

The Routine Phase provides ongoing support through context specific monthly and annual check-ins designed to maintain the established lifestyle and plans from previous phases, including medical advocacy,  medication management, and transportation assistance.  This continuous service is offered in the form of a monthly membership that can be cancelled at any time.

The Legacy Phase is dedicated to preserving and sharing your life story in a meaningful and lasting way.  Through a multimedia project, we capture personalized vocal and handwritten messages to loved ones alongside of photography, milestones, and values-centered storytelling.  The end result is a cherished gift perfect for ceremonies and designed in a format to be utilized as a hallmark in a home setting for generations to come.
